Program Overview
Who does the Trellus team consist of?
Trellus Health offers you remote access to a dedicated team consisting of Resilience Coaches, Registered Dietitians and Nurse Educators. We give you the tools, team, and technology to get you on track and keep you on track and in charge of your health between your doctor visits.
Does everyone work with a coach?
Yes. our highly skilled resilience coaches work alongside members throughout their Trellus Health journey. All of our coaches are trained on our scientifically proven approach to ensure you have the highly personalized tools, training and support needed to overcome any adversity and barriers that come your way and help you acheive your health goals and live life to the fullest. Through direct messaging and video, our coaches are at your fingertips when and where you need them to ensure you are never alone in your resilience roadmap to health and wellness.
What is empowered self-management
We know that managing the various aspects of your chronic condition - from tracking and managing symptoms, handling medications and procedures, maintaining your emotional well-being, communicating with providers, and getting adequate activity and nutrition takes patience and persistence, and can often be overwhelming. While the term “self-management” implies you are alone in the journey, our experienced resilience team will be by your side to help empower you to focus on the best self-management strategies for your particular situation, saving you time and effort so you can focus on other things you care about.
What does the resilience assessment do?
The free resilience assessment helps us understand where you are on your IBD journey so that we can better customize your very own roadmap to resilience for your mind and body.
What does a roadmap entail?
The entire series of assigned playbooks, resilience team visits, and disease specific notifications that support individuals member’s needs over time with Trellus to and desiged to ensure members manage their disease with confidence and take charge of their health.
What are playbooks?
The whole of assigned courses, compiled by the resilience team. Example: Mastering the Brain-Gut Connection; Mastering Your Worry, Managing Fatigue Playbook. Each playbook consists of small, digestible lessons, when combined together make up a playbook.
How long does the Trellus Program last?
Program length varies depending on your unique health needs and goals, and your resilience team will work with you to design the ongoing support you need to continue to thrive throughout your Trellus Health journey.The Trellus RiseTM program is deisgned to help you build the resilience and skills you need to overcome any barrier to achieving your health goals and the Trellus ThriveTM is designed to keep you there.
Do you replace my doctor?
We do not replace your doctor. While you may choose to work with us directly, we encourage collaboration with your doctor to ensure we address your health and wellness needs.
How do I connect you with my doctor?
After you sign up, with your permission, our team will contact your GI doctor's office to inform them about Trellus Health and how to work with us on your IBD journey.
Where is my personal information stored?
We take protecting your data seriously. You can rest easy knowing your information is secure and not being shared outside of our care network.

How much does the Trellus Health membership cost?
If you’re signing up through your employer or health plan, the cost of the program is fully covered. To sign up, visit the custom Trellus Health web page for your specific employer or health plan. If you're not sure of your URL, contact your HR or member services department, or contact Trellus Health by email at [email protected], or by phone at (800) 989-5503. If you’re signing up as an individual, it is $49/month. Depending on your HSA or FSA plan, health coaching could be a covered expense so we encourage you check with your employer.
What if I want to cancel my membership?
We really think you will benefit from continuing our program, but we understand that sometimes circumstances arise that can make it difficult to fully commit. If you would like to cancel your account for any reason, please send us an email at [email protected]
Teenage Population
How can Trellus support my teen?
Supporting a teen with IBD requires a unique approach. Trellus helps build confidence and independence so that your teen learns how to manage their disease, advocate for themselves, overcome life challenges, and thrive despite their diagnosis.
My child is a minor, will I need to give them permission to join?
Parent or guardian consent is required for all members under 18 (currently the youngest eligible age is 14 years old). You will be asked to provide consent at the beginning of the Resilience Assessment
Who will my teen be communicating with?
Your child will have the opportunity to meet virtually with their dedicated personal Resilience Coach, Registered Dietician, and Nurse Educator. They will also have unlimited direct messaging capabilities with these experts.
Do I need to be present during the virtual visits with my child?
You do not need to be present during the virtual visits. In fact, Trellus is focused on encouraging self-management and resilience. Both of these are strengthened when we feel responsible and in charge of our own wellbeing. While you are welcome to join, we will focus our attention on your child and encourage them to participate throughout the visit.
Will I have access to my child's account
Your child may choose to share their log in information. However, we encourage communication between you and your child regarding their wellbeing and we support this relationship as we help prepare them throughout their IBD journey and transition from pediatric to adult care.
What does "transition of care" mean?
Transition of care refers to the process of preparing one for the transition from pediatric to adult care. Trellus Health understands the value of self-management skills and wants our younger members to be equipped with the tools to transition to adult care without dependence on their parents, but rather on themselves. Our scientifically proven program will not only equip our teenage members to learn self-management skills, but also guide parents in supporting their child throughout the Trellus journey.
