UnitedHealthcare has teamed up with Trellus Health® to offer Trellus Elevate™, a clinically proven inflammatory bowel disease (IBD) program designed to help you manage the impact of the disease on your daily health and live your life to the fullest. Eligible UnitedHealthcare members residing in New York (NY) or Connecticut (CT) with Crohn’s disease or ulcerative colitis can gain full access to the Trellus Elevate™ program at no additional cost.
Trellus Elevate™ is a virtual program designed to work alongside your current management plan that includes:

- 1-on-1 virtual coaching with a dedicated team of specialists
- Emotional, social, and nutritional support
- Engaging learning modules and health management skills
